Clinique dentaire Trujillo et associés is required to collect the personal information necessary to provide dental care by means of the attached conﬁdential medical questionnaire, which is presented to you for signature.

The conﬁdential medical questionnaire is kept in your dental record, and your dental record is kept in file cabinets to which the public does not have access /or locked.

The conﬁdential medical questionnaire is kept in your dental record, and your dental record is kept on digital media, access to which is controlled by means of access codes.

Only nursing staff will have access to your health information; administrative staff will only have access to the information they need to carry out their duties (billing, appointment scheduling, insurance transmission).

Retention period for personal information

Your dental record is kept for five years following the last registration or insertion in the record, in accordance with the law. At the end of this period, it will be destroyed in order to preserve its conﬁdentiality.
